Cause of Death:
 Died in Aircraft Accident, while on patrol with No. 4 Squadron Royal Air Force, the R.E.8 aircraft #B734, carrying Girdlestone and his observer, 2nd Lieutenant Ronald Homersham, collided mid-air with another squadron aircraft (R.E.8 D4833). The collision occurred during poor weather conditions on the Western Front, severely damaging both planes and killing all four crew members involved.
